Donation Statement

If you would like a statement of your donations for the 2024 tax year, please call the parish office at (904) 284-3811.   If you are not registered, we will not be able to provide you with a contribution statement.  Parish registration forms are in the kiosk in the narthex, on our webpage, or may be obtained at the Parish Office.
